Your existing Fallout 3 saves will work with Fallout 3: GOTY edition.

Does this install the add-ons or run the entire thing from the disc?

The GOTY edition comes with 2 discs, one for the DLC, which is a one time install on your hard drive, and another copy of the original Fallout 3 game.  The DLC is 2.2 GB by the way.
