Clock Tower for Playstation, I remember the first time I played it, scared the hell out of me.
Call of Cthulu: Dark Corners of the Earth was pretty good too.
I think you sould get Eternal Darkness: Sanity's Requiem for the GameCube. That game is creepy

Manhunt and Condemned Criminal Origins are both pretty good choices just dont play Manhunt 2 -- its terrible
I love horror games! It's a shame that they've become so scarce recently.
FPS horror games aren't the greatest, but I was actually very impressed with Condemned. It's a good game. The sequel wasn't quite as creepy IMO, but it's good too.
I've heard a lot of people talk about Dead Space. I've had it on my shelf for about a month or two but I haven't got around to playing it yet.
The Fatal Frame and Silent Hill series are both very solid. Fatal Frame 2 and SH2 are the best of their respective series. Stay away from SH: Homecoming, though. Wasn't too impressed.
Clock Tower (SNES and the PS1 Sequel) are also great "hide and seek" horror games with great stories. Clock Tower 3 was average. Avoid CT: The Struggle Within / Ghosthead like the plague.
Haunting Ground is an excellent game. Very similar to CT3 but MUCH better.
Some of the Resident Evil games are very decent, but none of them are scary IMO. They do have their moments, though.
Avoid games like Rule of Rose and Obscure (mainly its sequel, Obscure: The Aftermath). Average horror games and not scary. Rule of Rose has an excellent story and soundtrack, but it's probably more fulfilling to just read about it online (or check it out on Youtube).
Dead Space
"Back in the day" the first Resident Evil was creepy as all get out. If you didn't jump when you first encountered the dogs, you aren't human.
But for actual scary? The Fatal Frame games are good for that :)

If you have a Wii, and have not been spoiled to the details of the game, get Eternal Darkness (Gamecube). Don't dare look up why it's scary. The less you know the better your experience will be. It was one of the best Gamecube titles released.
Condemned, Eternal Darkness, Dead Space are all good answers.
The correct answer, however, is Thief: Deadly Shadows.
I know I'm late but it seems everyone already agrees with me. I got Dead Space for 300 points with no previous interest. It was and instant cheap trade and I had a lot of points sitting around. I won't be trading it back out.
Dead Space is a great choice. I sadly just had to activate it, as it was giving me motion sickness. (A first for me.) It was a solid game and I was enjoying it, but oh well.
One of the only games that seriously scared me was System Shock 2. And I think Bioshock has some pretty creepy stuff too. I know the Big Daddy freaked me out the first time I saw him, and I LOVE the audio design in that game.
All time has got to be REmake. The only horror type game I've played lately, is Dead Space. So I guess that wins by default.
F.E.A.R. and Doom 3 probably, just for the jump at you moments. My wife laughed her ass off when I about threw my mouse while playing Doom 3 and one of the demons in a glass container moved when I was up close.
The first 30 minutes of Singularity had me freaked out.
